About the Role
Clarvida is seeking an ACT (Assertive Community Trement) Case Manager to support adults experiencing severe mental health challenges. In this role, you will partner with clients to develop and implement personalized service plans, focusing on areas like housing, employment, therapy, medical care, and substance use support. You will work directly with clients in their communities to improve their quality of life and facilite their recovery journey.
As an ACT Case Manager, you will collabore with a dediced team, provide hands-on support for daily living skills, and connect clients to necessary resources. You will also help track progress through home visits, offer transportion, and contribute to team-based client goals.
Responsibilities
● Collabore with clients to cree and implement personalized service plans
● Provide hands-on support in daily living skills, including budgeting, grocery shopping, meal planning, social skills, and coping stregies
● Connect clients to community resources and services
● Conduct regular home visits to track progress and offer support
● Collabore with the ACT team to help clients achieve their goals
● Provide transportion as needed
● Rote on-call responsibilities every 6 weeks (virtual or in-person)
● Provide hands-on support in daily living skills, including budgeting, grocery shopping, meal planning, social skills, and coping stregies

Requirements
● Bachelor’s degree in Psychology, Social Work, or a reled field
● Professional and courteous communicion (both verbal and written)
● Knowledge of and sensitivity to culturally diverse and special-needs populions
● Valid Georgia Driver’s License and clean Motor Vehicle Record (MVR)
● Clean Criminal Background Check (no conviction or credible evidence of abuse, neglect, or sexual exploition of children or adults)
